Do I need a referral from my primary care physician?

Most insurance companies Do Not require a referral. Just call the phone number on your insurance card to find out.

I received an explanation of benefits; is this my bill?​

No, this is not a bill. This letter is to inform you of the ammount a procedure, test, or treatment cost. This is often sent out to the patient when their insurance company is billed. Your insurance company has already determined your co-pay, deductable, and coverage.

Once your insurance company has paid, you'll recieve an invoice if there is a remaining balance.

What Health Insurance is Accepted?

Great River Pain Center accepts all major insurance coverage. If you are unsure if we are in network with your insurance company give us a call. We also offer self pay options as an alternative for underinsured or out of network patients.

What should I bring to my appointment?

Always bring your insurance card, id or drivers liscense, and your copay. We may not be able to get you in without these three things.

Also, bring any important documents such as X-Ray or MRI images with you. The office may ask you to bring other items as well when you schedule your appointment
